To find out if you are eligible for a Home Care Package, call My Aged Care on 1800 200 422 or visit  https://www.myagedcare.gov.au/

The My Aged Care contact centre will ask you a series of questions to determine if you need an assessment by an Aged Care Assessment Team (ACAT) .

If this is the case then the ACAT Team will make an appointment with you for an in-home face to face assessment. During the assessment the ACAT will determine if you are eligible for a Home Care Package and which package level best meets your care needs.

There are 4 different levels of Home Care Package available and each level provides a different level of funding.

When you have been approved for a Home Care Package,you will enter the national queue/waitlist to get a HCP Package assigned.

Once you get a package assigned you will receive a letter from My Aged Care with a referral code.

You will need to give this referral code to the Service Provider of your choice so they can access your approval.

The time from being approved for a Home Care Package to being assigned a Package can be long and you might want to use it to contact different providers to learn what they can offer you and what their charges are. Different Providers have different charges and you should also ask about their exit fee.
